He went on filling the relevant form as per the applicant's answers to his queries.

O several dates!" "O Mr; what do you say?" "I am honest and let me say it correctly.His one leg died (was amputated) on 13 February 1940; his eyes died (lost eyesight) on 10 January 1945); his ears died (turned deaf) on 9 January 1955; his one kidney died (was excised) on 6 March 2003.
